Lower back fusion surgery and back to work with a "no overtime" restriction reduced from 30% to 20% MAAW

Award: Reduced from 30% man as a whole to > 25% man as a whole

Accident: Lifting a lube pump felt back pain

Injury:  Low back pain with radiating pain into R leg.  Surgery required: posterior lumbar decompression at L4, posterior lumbar interbody fusion at L4/L5, pedicle screw fixation at L4/L5, harvest of autologous bone graft and placement of structural allograft to include 9mm spacers.  After surgery, some foraminal encroachment by disc bulge, or scarring, at L4-L5.

Restrictions: No more overtime; just 40 hours a week (but P asked for this restriction to be removed for financial reasons).
