Award: 19(b) hearing. Medical treatment and TTD awarded.
Accident: P was working as a credit counselor for office in Springfield. P completed meetings with clients, then headed back to Urbana where P resides. While he was driving back from Springfield, P was in car accident. After the accident P felt immediate left foot pain, shoulder pain, low back pain and neck pain.
Injury: L5-S1 small midline disc herniation with radiculopathy.
